Birth Certificate Forms
If you are at least 20 weeks pregnant and plan to deliver your newborn at Cheyenne Regional Medical Center, we ask that you complete a birth certificate form (also called the Mother’s Worksheet”) either prior to arriving at the hospital or during your stay in the Mother-Baby Unit.
The Mother’s Worksheet is sent to the State of Wyoming Vital Records Department and used to create the birth certificate.
After the delivery of your newborn, a Health Information Tech will visit your room to review your completed worksheet or to assist in completing missed questions.
