Employee CareFund Program Application

The CareFund Program provides eligible employees with up to $3,000 in financial assistance that does not need to be repaid. Please review the full Hardship Program policy on DocTract on the Intranet (Policy 4.10.005). To qualify, an employee must have experienced one of the following circumstances within the three months prior to the application date: an unplanned serious health condition; the need to care for a family member with a serious condition; a financially devastating tragedy or unforeseen disaster; or other extenuating circumstances, which will be considered on an individual basis. In addition, employees must meet certain eligibility requirements. They must have completed at least 12 months of consecutive service, and PRN employees must have worked a minimum of 200 hours within the past six months. Applicants must also have no corrective actions within the past 12 months and must not be eligible for short-term or long-term disability benefits or workers’ compensation.
