Biography
Travis Polk, NP, is a cardiology provider with a passion for caring for patients across the full spectrum of heart health, with a special interest in heart failure. Travis has been in healthcare since 1998. He has spent the majority of his career serving patients in emergency medicine, trauma outreach coordination, nursing home care, corrections and cardiology.
Travis chose medicine because he genuinely enjoys connecting with people and finds purpose in helping others. “I’ve always loved the science and wonder of medicine,” he says, “and it became a natural way for me to serve people in a meaningful way.”
Travis believes great care starts with listening. “Everyone has a story,” he shares. “Even if stories sound similar, they’re all unique. I want to hear your story and work together to build a plan of care that fits your life as it continues to unfold.”
Outside of work, Travis enjoys spending time with his family and is active in his local church, where he plays guitar and helps lead worship. In his free time, he likes rehabbing homes, woodworking, attending sporting events and keeping up with fantasy football.
