Get With The Guidelines®– Heart Failure is an in-hospital program for improving care by promoting consistent adherence to the latest scientific treatment guidelines. Numerous published studies demonstrate the program’s success in achieving significant patient outcome improvements. Program participation requires entry of baseline data into the Patient Management Tool™, an interactive online assessment and reporting system.
Gold Plus Quality Award is an advanced level of recognition acknowledging hospitals for consistent compliance with quality measures. Additionally, CRMC received the Association’s Target: Type 2 Diabetes Honor Roll™ award. To qualify for this recognition, hospitals must meet quality measures developed with more than 90% of compliance for 12 consecutive months for the “Overall Diabetes Cardiovascular Initiative Composite Score.”
