Q: Do you have free parking at your facility?
Yes.
Q: Do I need a referral?
No.
Q: Who are your participating insurance carriers?
Medicare, Medicaid, Blue Cross Blue Shield, Great West, and Altius (call us to confirm before you appointment)
Q: Are payment plans available?
Yes.
Q: Do I need to pay for my procedure/test prior to being seen?
No.
Q: Do you accept personal checks?
Yes, however they must be printed with your name and address.
Q: Can I pay by credit card? Which ones do you accept?
Yes, credit card payments are acceptable. We accept MasterCard and Visa.
Q: Do I have to pay my co-pay at the time of service?
Yes.
Q: Do I need a referral if you do not accept my insurance carrier?
No.
Q: What do I need for my first appointment?
You will need to bring your insurance Card(s) and a photo ID to your first appointment.
Q: How long will my first appointment take?
Your first appointment will last approximately 30 – 45 minutes.
