Q: Where should I park?
The patient parking lot and entrance are located on the north side of the building.

Q: Do I need a referral to see the endocrinologist?
No, although we highly encourage a referral as some insurance companies may require it. Please check with your insurance provider prior to your appointment

Q: Do I need a referral to see the certified diabetes care and education specialist?
Yes.

Q: Who are your participating insurance carriers?
Medicare, Medicaid, Blue Cross Blue Shield, Great West, Tricare, Veterans Administration, Aetna, United Health Care and UMR, to name a few. Please verify with your insurance company that your benefit plan covers our clinic and services.

Q: Are payment plans available?
Yes. To get set up on a payment plan, please contact our health system’s Patient Financial Services by calling (307) 996-4777, Monday through Friday, 8 a.m. – 5 p.m.

Q: If I do not have insurance, what am I expected to pay at the time of service?
If you do not have insurance, the minimum amount due at the time of service is $180 for a new patient and $120 for an existing patient or for a follow-up visit. The collected amount will be applied towards the charges incurred for that visit.

Q: Do you accept personal checks?
Yes, but they must be printed with your name and address.

Q: Can I pay by credit card? Which ones do you accept?
We accept MasterCard, Visa, Discover and American Express.

Q: Do I have to pay my co-pay at the time of service?
Yes.

Q: What do I need for my first appointment?
You need to bring your insurance card(s) and a photo ID to your first appointment.

Q: How long will my first appointment take?
Your first appointment will last approximately 40 to 60 minutes with the endocrinologist and approximately 60 to 180 minutes with a certified diabetes care and education specialist.

Q: How do I request a copy of my medical records?
If you would like to receive a copy of your medical records or have medical records sent to another party, please complete an authorization for release of information.
